Diabetes Self-Management Information Series for Primary Care

Diabetes Tasmania offers practical diabetes self-management resources to support health professionals when engaging with type 2 diabetes patients. These pictorial and interactive information sheets will assist patients in developing their self-management skills and knowledge.

Whether newly diagnosed or in need of a review of their diabetes care plan, each topic information sheet provides a framework for learning and links to further information.   

How to use resources

1. Ensure patient registered with NDSS regardless of treatment or management plan.

2. Together with patient, choose appropriate topic(s) to discuss. Provide patient with printed or electronic copy.

3. Refer patient to Diabetes Tasmania for additional education and support or talk with us about organising a multi-disciplinary consult with one of our diabetes nurse educators or dietitians


How to refer a patient

Diabetes Tasmania offers free of charge consultations with a diabetes nurse educator and/or accredited practising dietitian. Appointments are currently available via telehealth.
